What We Offer

  • At Kelly Jacobs Care, we provide in home Daily Living Supports to help participants manage everyday life with dignity, comfort, and confidence. Funded through the NDIS Core budget under the Assistance with Daily Life line item, these supports can include personal care, meal preparation, domestic assistance, and help with daily routines in the home, all tailored to the participant’s goals, preferences, and support needs.

  • At Kelly Jacobs Care, we provide Community Access supports to help participants safely and confidently engage in life outside the home. Funded through the NDIS Core budget under Assistance with Social and Community Participation, these supports can include assistance to attend appointments, go shopping, join social or recreational activities, access community spaces, and take part in meaningful day to day outings that align with the participant’s goals, interests, and individual needs.

  • At Kelly Jacobs Care, we provide Transport Assistance to help participants get to appointments, community activities, shopping trips, and other important outings safely and reliably. Where transport funding is included in a participant’s NDIS plan, this support is funded through the Core budget and is designed to make community access and everyday participation easier, more comfortable, and more consistent.

Our Process

Meet and Greet

Intake and Onboarding

You’re part of the process. We keep communication open and decisions shared. This stage is where information about you is gathered to ensure the right staff matching, the right supports are available and we remain compliant with the NDIS.

Plan with Purpose

This stage is where we design your supports with you. This is our Support Planning stage, you may also know this as your Care Plan. This ensures that we strive to help you achieve the goals you set for yourself at the pace you need.

Supports Begin

When we deliver, it’s not just a another shift, or another day. Each day is meaningful, intentional and deliberate. We are here to make each day easier, smoother and to be there to help you manage when it feels like you aren’t able to. We are here.
