We provide a range of NDIS supports designed to help participants live with more independence, confidence, and connection. Our services include Daily Living Supports in the home, Community Access, and Transport Assistance, all tailored to each participant’s individual goals and support needs.

Not at this stage. Kelly Jacobs Care is currently unregistered and is undergoing the NDIS registration and audit process. In the meantime, we can support participants where this aligns with their plan management type and individual funding arrangements.

Yes. We understand how important it is to feel comfortable with the people supporting you. We can arrange an initial meeting so you can get to know us, ask questions, and make sure the support feels like the right fit.

Yes. We are happy to speak with participants, family members, nominees, support coordinators, and other relevant people involved in the participant’s care and decision making.

We know consistency matters. Wherever possible, we aim to provide regular support workers so you can build trust, feel more comfortable, and have support from someone who gets to know you well.

Not at this stage. While we may be able to support some participants depending on their needs and circumstances, we are not currently offering services that require a registered provider where this is mandatory. As we progress through our registration and audit process, this may change.

Not at the moment. High Intensity Supports can only be delivered by appropriately trained workers under a registered NDIS provider, so this is not something Kelly Jacobs Care is offering just yet while we undergo registration.
